Example in a Contract
Eligibility for Family Medical Leave. An employee is eligible for up to 12 weeks of unpaid leave in a 12‑month period if (i) the employee has been employed for at least 12 months, (ii) the employee has worked at least 1,250 hours during the preceding 12 months, and (iii) the employee works at a location where the employer has 50 or more employees within a 75‑mile radius. The employee must provide at least 30 days’ advance notice when the need for leave is foreseeable, and must furnish a medical certification within 15 days of the employer’s request.
